|  | |  | | Patient Number 1: Breast Implant Removal | | Actual Patient -- Results vary from patient to patient |  | | Age: 46 | | 46 year-old woman who had lower-300cc range saline implants placed subglandularly (over the muscle) approximately 4 years earlier, resulting in soft, fairly well-positioned implants, and a smaller D-cup bra size. She ultimately felt this was a bit large, however, and wished to have them removed, mostly due to weight/discomfort issues, and the fact that she was OK with a B/C cup bra size. She did recognize that the implants (and general aging, having also had 3 children) had led to a fair amount of skin excess, which would likely have to be addressed with a breast lift, especially in the face of the "deflation" effect that would result from removing the implants.
This is indeed what I recommended, that she undergo a formal mastopexy, with placement of small drains in the pockets to help them collapse. |  | |  | | Patient Number 2: Breast Implant Removal | | Actual Patient -- Results vary from patient to patient |  | | Age: 54 | | 54 year-old patient with implants present for approximately 12 years; developed moderate capsular contractures (but no clear rupture), and desired outright removal of the implants. She was advised that she of course would be smaller, but that she had relatively mild skin excess or nipple ptosis (drooping), and therefore a formal mastopexy would not be necessary. She was advised however, that to collapse down and heal the current capsules that had developed in the implant space, that she would need an anterior capsulectomy following explantation on either side, and the placement of small drains for several days to minimize the chances of developing a seroma.
This indeed was performed as an outpatient under light general anesthesia, and the patient did very well, developed no complications, and was left with a very acceptable cosmetic result, in a smaller B-cup range (photos are from approximately 3 months post-op.) | | Cup Size Before: C | | Cup Size After: B | | Photos taken: 3 months after surgery |  | | Email Dr. Steinwald |
